PURE PICKLEBALL LIABILITY WAIVER

By Booking any session or participating in any activities at Pure Pickleball, you agree to the terms outlined in this Liability Waiver and Release Agreement. Please read this document carefully before signing.

1. Acknowledgement of Risk

I understand that participating in pickleball involves inherent risks, including but not limited to:

• Slips, trips, and falls.

• Collisions with other players, equipment, or court surfaces.

• Muscle strains, sprains, or other injuries from physical exertion.

• Risks associated with pre-existing medical conditions.

I confirm that I am voluntarily participating and assume full responsibility for any risks, injuries, or damages that may occur.

2. Medical Fitness and Consent

I certify that:

• I am physically fit and have no medical conditions that would prevent safe participation.

• If I have any medical concerns, I have consulted a doctor before engaging in pickleball.

• In case of an emergency, I consent to receive necessary medical treatment and assume any costs incurred.

3. Release of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge Pure Pickleball, its owners, staff, volunteers, and affiliates from any claims, liabilities, or damages arising from:

• Personal injury or illness.

• Loss or damage to personal property.

• Any accidents occurring before, during, or after participation.

This waiver does not exclude liability for negligence resulting in death or serious injury where such exclusion is not permitted under UK law.

4. Code of Conduct and Safety Compliance

I agree to:

• Follow all safety rules, venue policies, and instructions from staff.

• Respect other players, equipment, and facilities.

• Play responsibly and report any hazards or injuries.

Failure to comply may result in removal from the venue without a refund.

5. Photography & Media Consent (Optional)

I grant permission for Pure Pickleball to capture and use photos/videos of me for promotional purposes. If I do not wish to be photographed, I will notify staff in advance.

6. Parent/Guardian Consent (For Participants Under 18)

If the participant is under 18, a parent or legal guardian must sign this waiver:
"I acknowledge and agree to the above terms on behalf of my child/minor and accept full responsibility for their participation."

7. Agreement and Signature

I confirm that:

• I have read and understand this Liability Waiver.

• I voluntarily agree to its terms.

• I am 18 or older, or my legal guardian has signed on my behalf.
